Having both Medicare and Medi-Cal benefits can be confusing. That's why Community Health Group now offers CommuniCare Advantage Cal MediConnect Plan, a comprehensive new plan that combines your Medicare and Medi-Cal benefits into ONE plan with one card. Better yet, CommuniCare Advantage Cal MediConnect Plan has added benefits at no extra cost to you.

Am I eligible for CommuniCare Advantage Cal MediConnect Plan?

If you live in San Diego County and you are receiving Medi-Cal and Medicare benefits (have both Part A and Part B), you can sign up for CommuniCare Advantage Cal MediConnect Plan. It's a simple way to combine your Medi-Cal and Medicare benefits into one easy to use health care plan.

Can I still go to my regular doctors?

As long as your doctors are part of our CommuniCare Advantage Cal MediConnect Plan provider network. CommuniCare Advantage Cal MediConnect Plan members will have access to many hospitals in San Diego County.

Does CommuniCare Advantage Cal MediConnect Plan include prescription drug benefits?

Yes. As a member of CommuniCare Advantage Cal MediConnect Plan , you will have the same drug benefits you receive under Medicare Part B and under the Medicare Part D prescription drug plan. Depending on your income the co-payments for drugs may vary from $0 to $8.25 per drug: most generic drugs have $0 co-payment. After your yearly out-of-pocket costs for drugs reach $4,950, you won't have to pay any more co-pays for drugs.

Does CommuniCare Advantage Cal MediConnect Plan have deductibles or co-pays for medical services?

No. You can enjoy the added convenience and ease of CommuniCare Advantage Cal MediConnect Plan with no extra costs. There are no premiums, no deductibles, and no co-pays. You pay zero dollars out of your pocket for doctor's visits, emergency services or hospitalization.

What are the added benefits of CommuniCare Advantage Cal MediConnect Plan?

In addition to your current Medi-Cal and Medicare benefits, CommuniCare Advantage Cal MediConnect Plan includes:

  • Non-emergency transportation for 36 one-way visits to the doctor per year.
  • Worldwide emergency services (excludes transportation back to the U.S.).
  • Monthly gym membership with pre-authorization from plan nurse or case manager
  • Up to $200 for eyeglasses (frames and lenses) or up to $200 for contact lenses once a year.

You will also have access to a Customer Service Representative 24 hours a day, seven days a week!

CommuniCare Advantage Cal MediConnect Plan is a health plan that contracts with both Medicare and Medi-Cal to provide benefits of both programs to enrollees. Enrollment in CommuniCare Advantage Cal MediConnect Plan depends on contract renewal.

Medicare beneficiaries may not be able to renew their contract every year for these plans. There are many reasons for this. They include the following:

The Centers for Medicare and Medicaid Services (CMS) may not renew their contract with CommuniCare Advantage Cal MediConnect Plan or your plan sponsor to provide Medicare and Medi-Cal benefits.

CommuniCare Advantage Cal MediConnect Plan or your plan sponsor may not renew their own contract with CMS to provide Medicare and Medi-Cal benefits.

CommuniCare Advantage Cal MediConnect Plan or your plan sponsor may need to offer Medicare and Medi-Cal benefits to a smaller service area.

You must live in the service area for the Cal MediConnect Plan.

Copays for prescription drugs may vary based on the level of Extra Help you recieve. Please contact the plan for more details.
