Ideal candidate will conduct home and on-site visits. This will require applicant to: • Perform comprehensive in-home assessments for members by: o Reviewing all their needs including: medical, social/psychological, financial, caregiver support and environmental safety. o Documents recommendations for additional screening tests to identify and/or support suspected, but not proven, risk-adjusted diagnoses. • Establish and maintains effective relationships with members and/or legal representatives. • Assist in resolution of member complaints and/or grievances. • Work some evenings, weekends, and holidays. • Attend meetings outside of facility or normal business hours. • Have a valid driver’s license, reliable transportation, and automobile insurance. Preferred attributes of ideal candidate: Education • Current, unencumbered MD license in California. Experience and Skills • One year, post graduate hands-on clinical experience. • Bilingual, English/Spanish preferred.
