Community Health Group’s Participation in San Diego County’s Community Health Assessment/Community Health Improvement Plan (CHA/CHIP)

Starting January 1, 2024, Community Health Group began meaningfully participating in San Diego County’s CHAs/CHIPs. Community Health Group participates in several collaborative meetings with the County of San Diego, Health and Human Services Public Health Services, other Managed Care Plans, and the Hospital Association of San Diego and Imperial Counties. Additionally, Community Health Group contributes resources to support the CHAs/CHIPs.
